For school enrollment, a parent or guardian shall provide one of the following:
1. Curent, up-to-date immunization records; or
2. A completed and signed exemption form:  Certificate of Exemption

DAILY ACTIVE ISOLATION:  Berryhill Covid Data
 

"A" Symptoms "B" Symptoms
Fever >100.4 F Nasal Congestion/Stuffy Nose
Sore Throat Runny Nose
Cough Muscle/Body Aches
Difficulty Breathing Fatigue
Nausea or Vomiting Chills
Loss of Taste or Smell Nausea
  Loss of appetite
  Elevated Temperature <100.4F
  New Onset of Severe Headache
Recommend testing when:
A person has 1 "A" Symptom 
OR
A person has 3 "B" Symptoms
