Growing Up with Vaccines:  What Should Parents Know?

For school enrollment, a parent or guardian shall provide one of the following:
1. Curent, up-to-date immunization records; or
2. The exemption process is online and can be accessed at this link: https://imm-public.osdh.immytech.com/.  Once you have received the approval from the state, please submit this to me at Tara.wallace@berryhillschools.org  For further clarification of the exemption process, please see attached form.  Immunization Exemptions

Child Nutrition and Physical Fitness Program Review

It is the intent of the board of education that parents, students, child nutrition employees, teachers of physical education, school health professionals, and the general public participate in the development, implementation, and periodic review and updating of this policy.  Any recommendations must be submitted in writing to the Office of the superintendent, 3128 S. 65th West Avenue, Tulsa, Oklahoma 74107.
